You don't cut out any milk feeds when weaning. You still offer them as normal and when your LO starts to cut them out herself then you follow her lead and cut back. Milk remains the most important source of nutrition till your baby is one so it is important to keep it in her diet for that reason and let he have as much as she wants/needs.

As for water let her drink what she wants with meals. If she is thirsty she will drink it, if she isn't she won't. Unless your tap water isn't drinkable for some reason then there is no need to use boiled and cooled water, you can use it straight from the tap.
