Blinkin heck. It sounds like you're in a bit of a pickle... I was wondering about perhaps a cleft palate, then you mentioned you've seen a paediatrician. Has anyone examined his mouth? Is there any possibility of a basic problem like thrush causing him pain on his gums? Early teething?
No wonder you're distressed (((hugs)))
I take it you have tried lots of different positions. He might be uncomfortable.. and you say it was a normal delivery. Quandry quandry quandry...
Have you tried a nice warm bath, darkened room, and reintroducing him to the breast like it is his first ever feed? Sit in the water with him dipped in in front of you, scoop him up gently onto your chest and leave him resting there, warm dry towel draped over him. Lie for about half an hour (max) and see if he starts to search for the breast. He might even latch on himself. f not, you could gently suggest it yourself. Or he might just go to sleep lol This theory is all about making it relaxed and close and comfortable...
Are you feeding on demand or are you sticking to set times?
I'll have a wee look at a book I've got. It has a chapter on being fussy at the breast. It's LLL though, so I don't imagine I'd be telling you anything new.
Have you tried having a duvet day with him? Nice warm room, sleeping together and waking for feeds and sleeping during them etc. Might relax things a little.
Have they checked his wee tummy etc?
